I have noticed recently that I have blood in my seamen, I have had severe cramp like pain in my testicles for the past few months, the pain is getting to a stage where it is becoming unbearable, what could this be?

The possible causes of hematospermia or blood in the semen are inflammation, infection, obstruction, or injury occurring anywhere in the male reproductive tract. It could be an indicator of a problem in the urethra, epididymis, testicles, or prostate. Among the conditions where there could be blood in the semen are: prostatitis- inflammation of the prostate, sexually transmitted diseases and benign prostatic hypertrophy. Those are just a few of possible underlying causes, it is advisable to seek prompt medical attention because you have also been experiencing testicle pain for months.
The most common cause of testicle pain in adult men is epididymitis- inflammation or infection of the epididymis. Presence of stones in the lower ureter, fluid in the testicles due to conditions like spermatocele and minor injury to the scrotum may also result to testicle pain. You have to be examined by your primary care physician or a urologist to find out what has been triggering the pain. In general, evaluation may depend on the age and health history of the patient. A more intensive work-up may be preferred for older men, or for those who have recurring episodes of symptoms.
